ACRC Parent Support Group Childcare Registration

Please complete this form if you need childcare during an ACRC Parent Support Group meeting. Childcare is available for children with special needs and their siblings from 6:00–7:00 p.m. on the second Wednesday of each month. A separate form must be completed for each child who will attend.
